New requirement for HSIP receipt documentation submission
Effective July 17, 2023, it will be necessary to submit the Subject Information Excel template to document payments for all tier 1/3/5 HSIP requests.
This template has been widely used by many members of the research community for a number of years to submit HSIP documentation. If, however, you’re not familiar with the template, it’s easy to use and will replace the individual handwritten receipts and/or other documentation that have been provided to our office after payments have been distributed.
The new requirement is designed to improve the efficiency and accuracy of the HSIP documentation process. As a reminder, the template should include the following details:
Name (first and last)
Address (complete with street address, city, state, ZIP code, and country code)
Payment amount
Payment date
SSN (if tier 3 or tier 5)
By providing this information, you’ll help us ensure that all HSIP requests are processed in a timely and accurate manner. Our primary goal is to make the transition to the new documentation process as seamless as possible for the U-M research community.
