HealthStyles Health & Fitness - Initiation fee: $50. Annual membership fee: $300 (EFT debit $25 per month) or $90 quarterly. Direct family members pay $50 initiation fee, $558 annually or $166.50 quarterly. Groups of 2-5 members each pay $50 initiation fee, $240 annually or $75 quarterly paid in advance. Other services include massage, personal training, and nutritional analysis.

Offers physical therapy, occupational therapy, speech therapy, and contractual arrangements for all therapy provisions. The patients are considered our priority, and there is constant communication, throughout the course of treatment, between health care personnel involved in the rehabilitation process.
